...a healthy lifestyle program to prevent diabetes
The Adelaide Hills Division of General Practice is pleased to announce the introduction of "Reset your life", your local accredited Type 2 Diabetes prevention program. A doctor's referral is necessary for this program, and programs will be run locally.
Landmark intervention studies in China, the US and Finland have found that sustainable lifestyle interventions in people at high risk of developing Type 2 Diabetes led to significant reductions in the incidence of diabetes - by up to 58%.
Under the Australian Government subsidised Reset your life program, patients in the 40-49 year old age group assessed as high risk of developing Type 2 Diabetes based on the AUSDRISK tool, and who have had diabetes excluded, can be referred to Reset your life under:

*Doctors may use the time-based item numbers (701,703,705 and 707) to refer to the LMP - nurse time is also included when calculating the time spent with a patient.
Reset your life will be a series of 7 group education and motivation sessions over 6 months including:
- What is pre-diabetes and diabetes
- Physical activity and healthy weight
- Healthy eating
- Prevention of cardiovascular disease
- Staying motivated, positive thinking and goal setting
- Stress management
Sessions are run by qualified facilitators, including various allied health practitioners. Feedback on individual patient participation and progress will be reported to the referring General Practitioner on completion of the program.
The Division will charge the patient a one-off co-contribution of a maximum of $50 incl. GST. For patients with a health care or concession card, this co-contribution is waived.
