The Adelaide Hills Division of General Practice provides a range of mental health programs to support GPs in the management/treatment of patients with mental health issues. This service is provided to low income patients at no cost.
To access these programs a GP Mental Health Treatment Plan (Item 2710 or 2702) will need to be completed and sent to AHDGP (via e-referral process or faxed to 8406 7760)
Once received, the referral is triaged and allocated to the most appropriate Program and Mental Health Clinician and an appoinment is made with the patient directly (patients are able to access 6 sessions initially). GPs will receive patient assessments and regular feedback ensuring good collaborative care. A GP Mental Health Treatment Review (Item 2712) can also be utilised to enable further sessions (6), if required.
Psychologists, Mental Health Nurses and Social Workers make up our team of Allied Health Professionals who deliver focussed strategies, including psycho-education, cognitive behavioural therapy and interpersonal therapy in individual counselling sessions.
Clinics are held in Mount Barker, Strathalbyn, Stirling, Nairne, Woodside and Gumeracha.
This free service is made possible by the support of State & Federal Government funding.
