The AHDGP now provides a Health Assessment fee-for-service program for General Practice.
Health Assessments for the over 75 years age group and Comprehensive Medical Assessments for aged care residents can be carried out on your behalf by Health Assessment nurses employed by AHDGP.
Our Health Assessment nurses are experienced in aged care and familiar with local and community services.
Patient assessments and referrals for services are sent electronically.
An advantage to General Practice is no requirement to employ additional Practice Nurses and reduce the need for workspace.
This service is a financially sustainable model of patient care. Currently the cost of a 75 plus Health Assessment and Comprehensive Medical Assessment is $100 plus GST.
